After many years of practice, retiring in 2012, I have been privileged to care for patients with Neurologic disorders. For your information, we are required to keep adult medical records for 10 years since last being seen (2010 or more recently).

 

If you are in need of medical records from that time frame,  please mail a letter of request for records to:

For processing, your letter of request should include:

• Your full name

• Your date of birth, 

• Address to where you would like your records sent

• The date of your visit if known

• Your full legal signature

• The date of signing

• Your phone number or your return address if we have a question

There is no charge for records requested for personal use or from your physician's office. Labor and Industry,  Insurers, and legal requests with appropriate authorization will be charged usual and customary fees.
